Forged Physical Therapy and Performance is a cash-based physical therapy practice. We do not bill insurance companies directly. Payment is due at the time of service unless otherwise agreed upon in writing.

You are financially responsible for all services rendered
Insurance reimbursement, if available, is your responsibility
We make no guarantees regarding insurance reimbursement

If you participate in virtual rehab coaching or performance training services, you acknowledge and agree that:
Virtual services may not be appropriate for all conditions
Certain assessments or treatments may be limited compared to in-person care
You are responsible for providing a safe, private environment during sessions
Virtual services are provided in compliance with state licensure laws and may only be available in states where the therapist is licensed
Additional consent forms may be required prior to participation in virtual services.
6. Online Programs & Digital Content
Any online programs, exercise plans, or digital content provided by Forged Physical Therapy and Performance are for educational purposes only unless explicitly stated otherwise.
Results are not guaranteed, and participation is voluntary. You assume full responsibility for your participation and outcomes.
7. Assumption of Risk
Physical therapy and exercise involve inherent risks, including injury or aggravation of existing conditions. By participating in any services—whether in-person or virtual—you voluntarily assume all risks associated with physical activity and rehabilitation.
You agree to follow all instructions provided and to stop any activity that causes pain, dizziness, or discomfort and report concerns immediately.
8. Cancellation & Refund Policy
Appointment cancellations, rescheduling, and refund policies are communicated separately at the time of booking. Failure to comply with cancellation policies may result in fees.
Digital products, online programs, or completed telehealth sessions are generally non-refundable unless otherwise stated.
9. Intellectual Property
